Attention RSVP Volunteers
As part of our Federal Requirements we need to update enrollment forms every 3 years whether you volunteer directly for a RSVP Signature Program or are placed at one of our partner sites. If you have not done so, please click here {link to Updated Enrolment Form 1-10 .pdf} to print a copy of the form. Please fill out completely and return to RSVP Office at 128 Providence St. Box 4 Worcester, MA 01604.
If you have renewed your driver’s license within the last 3 years we also require a copy in order to maintain supplemental insurance for you. Please include a photocopy of your new license or you can stop by our offices and we will make the copy for you.
